Application Requirement
To apply for food business license, it shall obtain the business license, registration certificate of legal person, registration certificate of a organization.The specified entity registered on business license serve as applicant of business license, including business entity, partnership business, sole proprietorship, individual business and branch office of business entity. If the department, state institution, orginization, private non-enterprise units apply for canteen, the specified entity registered on Certificate of Public Institution or business serve as applicant.
Required Materials:
1. Food Business License Application
2. Certificate of office space
3. The Statement of using of space
4. The original rental certificate of house should be verified and the copy should be provided.
5. Interior Plan
6. The list of operation equipment and tools used in food business.
7. The copy of business license (with stamp)
8. The copy of production license (with stamp)
9. The original ID card or passport of legal person and responsible person, and the copy should be provided.
10. Attorney's letter
11. The declaration form of food category
12. The management system of food safety
13. Operating process
14. The safety commitment of food safety
15. The original business license and its copy
16. The official seal
17. The certificate of food safety administrator
18. Health certificate
